Dear Parent/Guardian, New Wine Church is committed to helping every Christian establish a relationship with the Lord Jesus Christ. We are also committed to helping the youth of Lynchburg live a wholesome life by providing them a conducive environment for teenage development. Your son / daughter / ward / has indicated interest to be water baptized by the church. If you do grant him / her permission to do so, please kindly sign the consent form below.
